Medical and Tuberculosis Screening (Click Me!)

Cost: Varies by Insurance Coverage, Healthcare Provider

Instructions: The medical and tuberculosis screening must be completed by a licensed healthcare provider. The form requires both a brief medical exam to ensure physical wellness required to safely interact with youth and a Tuberculosis test which can be administered via PPD, Quantiferon, or x-ray tests. Generally, the medical and tuberculosis screening can be completed by the primary care physician; in urgent cases, clinics or Urgent Care centers may be able to complete the medical and tuberculosis screening document.

Please note that for the Medical/TB Clearance:

  • Both boxes on the form must be checked off
  • The form must have the signature and stamp of the provider
DC Criminal Clearance (Click Me!)

Documentation: Available at DC Metropolitan Police Station

Cost: DC and VA Residents: $42; MD Residents: $7

Instructions: Visit the DC Metropolitan Police Department located at 300 Indiana Avenue NW, Washington, DC 20001.

DC/VA Residents: take the elevators UPSTAIRS to the third floor to be fingerprinted for both the DC Criminal and FBI Clearances

MD Residents: take the elevators DOWNSTAIRS to the first floor and complete the DC CRIMINAL CLEARANCE ONLY

If you have another state that you are required to complete clearances, check to see if your respective state is able to run an FBI Criminal Clearance or whether it will need to be obtained from the DC Metropolitan Police Department.

Instructions:

Maryland: See respective form for list of locations for fingerprinting. Maryland residents are REQUIRED to complete the FBI Criminal Clearance with the Maryland Criminal Clearance, and therefore are not required to obtain a second copy of the FBI Criminal Clearance from the DC Metropolitan Police Department.

Virginia: See the attached link to generate your unique Form SP-167. When completing the form, please list yourself as both the requestor and the individual being searched. This will expedite the process by requiring only your notarization. Once you receive this background check in the mail, you must submit a copy to clearances@sashabruce.org in order for it to be processed, as a copy will not automatically be submitted to Sasha Bruce from VA CJIS.

Other States: State Criminal Clearances can often be requested via internet or snail mail; it is your responsibility as the [potential] staff member to research your respective state to find out and to complete the exact process. Any questions or concerns may be directed to the Human Resources Department.

FBI Criminal Clearance (Click Me!)

DC/VA Residents: See above information regarding DC Criminal Clearance

MD Residents: See above information for completing MD Criminal Clearance

Cost: Varies by Jurisdiction/Vendor

Instructions: Staff are only required to complete one FBI Criminal Clearance from EITHER the District of Columbia OR the State of Maryland. Maryland Residents are required to complete the clearance in Maryland; all residents of the District of Columbia and Commonwealth of Virginia are able to complete the clearance in the District of Columbia. Other states should check in with the Criminal Justice Information Services to see if an FBI Clearance is easily obtainable alongside of that State’s criminal clearance; otherwise, staff are able to complete the clearances the Metropolitan Police Department.

DC Child Protection Registry Clearance (Click Me!)

Documentation: See clearance attachments below; please note separate forms for NEW STAFF and CURRENT STAFF

Cost: Free to Submit (REQUIRES NOTARIZATION)

Instructions: Complete the attached form and notarize. Hand deliver to the Child and Family Services Agency, located at 200 I street SE, Washington, DC 20003, and send clearance receipt to clearances@sashabruce.org.

State Child Protection Registry Clearance (Click Me!)

Maryland:

Cost: Free to Submit (REQUIRES NOTARIZATION)

Instructions: Complete the attached forms and either hand deliver or mail to the Baltimore address or hand deliver to your local county Department of Social Services. Human Resources suggests hand delivering so that a receipt (and ability to follow up) may be obtained. Depending on your county of residence, this form may take UP TO EIGHTEEN WEEKS (4-5 months) to be returned to Human Resources; please complete it as soon as possible to minimize delay in the onboarding process.

 

Virginia:

Instructions: Complete the attached forms and deliver to the respective address on the form.

Mandated Reporter Certificate (Click Me!)

Instructions: Register and complete this online course (Click Me!) that covers basic child welfare laws and requirements, including situations in which SBY Staff are classified as Mandated Reporters of the Abuse and Neglected, Actual or Suspected, in the District of Columbia. Once the course is completed, ensure the Certificate is sent to clearances@sashabruce.org

Driving Record

Documentation: Varies by Jurisdiction

Cost:  Varies by Jurisdiction

Instructions: If you are unsure if your position will require you to transport Sasha Bruce Youthwork clients, please contact Human Resources. If you will be required, per your position, to transfer youth (to school, court appointments, doctors’ appointments, etc.), we will need a copy of your Five Year Driving Record as well as a copy of your Valid Driver’s License in order to add you onto our company vehicle insurance.

Drug and Alcohol Screening (Click Me!)

Documentation: None needed; once all other clearance documents are submitted, you will be registered by a member of the Human Resources team.

Instructions: COMPLETE THIS LAST. Once all other clearances are processed and returned to Human Resources, you will be registered for the Drug and Alcohol screening through HireRight. This process typically takes 24-48 business hours following your completion of the test, and results are delivered automatically and electronically to Sasha Bruce Youthwork. Once you complete your part of the registration (sent via email from HireRight), you will have five business days to complete the test.

Degree or Diploma (Click Me!)

Please provide a copy of your degree or diploma (highest level of education COMPLETED) to Human Resources to include in your clearance packet. Please note that due to licensing requirements, TRANSCRIPTS ARE NOT ACCEPTABLE DOCUMENTATION OF PROOF OF EDUCATION.
